Non clinical jobs for physicians use your medical judgment without direct patient care. The ones you can do from home include utilization review, insurance record review, medical writing, clinical informatics, expert witness work and AI training. Some need an active license and some only need the MD.
Here is what each path involves and what it asks of you, from professional bodies, real listings and doctors who made the move.
Where Physicians Work Outside Patient Care
Most physicians are in clinics and hospitals, but not all. The Bureau of Labor Statistics names government agencies, nonprofit organizations and insurance companies as nonclinical employers, and notes that some physicians choose medical research or public policy instead of patient care.
The AMA has profiled physicians who left clinical work. Their list of options includes:
- Medical education and education administration.
- Expert witness testimony.
- Pharmaceutical companies, which look for medical expertise.
- Insurance companies that need medical records reviewed.
- Policy and government roles.
- Media.
One physician executive quoted by the AMA said some insurance review roles require an MD but not a medical license.

Utilization Review and Physician Advisor
Physician advisors review whether hospital stays and services meet admission and coverage rules, and they talk through cases with case managers and treating physicians.
An Optum physician advisor listing, taken down in February 2026, shows what these roles ask for:
- Remote from anywhere in the US.
- Overnight weekday shifts, with weekends shared across the team.
- An MD or DO, board certification and a current, unrestricted license.
- At least 3 years in hospital-based practice.
- Applying CMS and payer rules on inpatient, observation and outpatient status, and managing length of stay.

Clinical Informatics
Clinical informatics is a physician subspecialty with its own board certification. The route into it has narrowed.
The American Board of Preventive Medicine (ABPM) ran a practice pathway from the 2013 through the 2025 exam cycles. From 2026, the route is a 24-month ACGME-accredited clinical informatics fellowship, and you need a primary certificate from an ABMS member board to apply. Pathologists apply through the American Board of Pathology instead.

Medical Writing and Education
Medical writers and editors work for research institutions, hospitals, medical education companies, publishers, drug and device companies, and health agencies. The New England chapter of the American Medical Writers Association says some of its members freelance and others work full time.
One physician chief medical officer told the AMA that education is a big player among nonclinical options.

Expert Witness Work
Physicians are called as expert witnesses in legal and administrative cases. The AMA Code of Medical Ethics sets the terms:
- Testify only in your area of expertise, with recent and substantive experience.
- Give an objective, independent opinion grounded in current science.
- Judge the standard of care as it stood when the events happened.
- Describe your credentials truthfully.
- Never accept a fee that depends on the outcome of the case.

AI Training
AI companies pay physicians to write realistic cases and grade how their models reason. On 2026-10-13, Mercor‘s public feed had remote physician roles for primary care, hospital medicine, oncology and cardiology, plus standing pools that invite doctors to projects as they open.
Most of these listings want current practice and an active US license. A few welcome final-year residents and fellows.
Our guide to medical AI trainer jobs covers what the work involves, the screening steps and each role's listed pay.

How to Make the Move
Physicians who have already done it gave the AMA this advice:
- Find people whose roles you want and ask them how they got there. One said he always answers emails about career paths.
- Start from your values and interests, then look for the role that uses them.
- Expect the jump to be harder without clinical experience behind you.
Physicians on Reddit add a warning for anyone thinking of medical school only for a nonclinical career. Teaching doctors usually needs a completed residency, and several said nonclinical pay is a hard way to clear medical school debt.
