Most remote pharmacist jobs sit away from the retail counter: reviewing prior authorizations for health plans, verifying hospital orders from home, consulting patients by phone or app, field roles for drug companies, and AI training projects. Most of the roles below ask for a Pharm.D. and an active license, and some tie you to a particular state.
Below is what each path involves, who it suits, and what pharmacists who have made the move say about it.
Where Pharmacists Work Today
Retail and hospitals still employ most pharmacists. The US Bureau of Labor Statistics counted 325,200 pharmacist jobs in 2025, split like this:
| Employer | Share of pharmacists |
|---|---|
| Pharmacies and drug retailers | 37% |
| Hospitals | 32% |
| General merchandise retailers | 6% |
| Ambulatory healthcare services | 6% |
BLS projects 5% job growth from 2025 to 2035. It expects retail demand to be limited as more people fill prescriptions online or by mail, which is part of why pharmacists look at other settings.

Managed Care and PBM Pharmacy
Health plans and pharmacy benefit managers (PBMs) employ pharmacists to decide which drugs get covered and how. The Academy of Managed Care Pharmacy (AMCP) estimated about 18,000 pharmacists worked for health plans and PBMs in its 2019 careers brochure.
AMCP describes the work as including these services.
- Prior authorization reviews.
- Drug utilization review, checking claims for interactions, duplicate therapy and dosing problems.
- Fraud, waste and abuse programs.
- Evidence reviews for the pharmacy and therapeutics committee that builds the formulary.
- Pharmacy benefit design and medication therapy management programs.
On Reddit, one pharmacist weighing a remote prior authorization contract with a large chain described shifts running into the evening, one weekend day, productivity metrics, and no promise the contract would turn permanent. They still saw it as a way into managed care.

Remote Order Verification
Hospitals that cannot staff a pharmacist around the clock send orders to a pharmacist working off site. ASHP issued its first guidelines for this kind of remote medication order processing in 2010, and the coverage at the time framed it as a way to cover nights and weekends.
Retail chains run the same model for prescriptions. On 2026-10-13, the Walgreens careers site listed hybrid retail and remote pharmacist roles in North Carolina, Missouri and Tennessee, plus work-from-home centralized services roles that required living in South Carolina or Delaware.
Pharmacists on Reddit who do this work describe a few things to expect:
- Speed and accuracy are both measured, and reviews lean on order counts.
- Covering several hospitals can mean holding licenses in several states and learning each hospital's systems and policies, so onboarding can take months.
- Hours of back-to-back verification at a desk can be draining.
- Some roles ask for a license in the pharmacy's state but let you live elsewhere in the US.
One remote hospital pharmacist wrote that people coming from hospital practice tend to fit the work better than people coming from retail.

Remote Patient Consults
Some companies pay licensed pharmacists to talk with patients from home. Aspen RxHealth, for example, describes a fully remote model where pharmacists use an app to consult patients on medication therapy management, adherence, chronic disease management and medication reconciliation, and choose when they work.

Drug Company Roles
BLS notes that pharmaceutical industry pharmacists work in marketing, sales, research and development, and clinical trials.
One industry role pharmacists ask about is the medical science liaison (MSL). An MSL writing for the American Association of Psychiatric Pharmacists describes it as a non-promotional medical affairs role that needs advanced training such as a Pharm.D. or PhD. It is field based: you work from home inside a territory and travel to meet clinicians.

Licensing for Remote Pharmacy Work
Every state requires a pharmacist license, and remote work does not change that. When the University of Minnesota Rural Health Research Center surveyed telepharmacy rules, the state board directors it interviewed generally agreed that a pharmacist should be licensed in the state where the service is provided.
Rules differ by state and by type of work, so check with the board of pharmacy in each state you would serve before you apply. This page is not legal advice.

AI Training Work for Pharmacists
AI companies pay pharmacists to check what their models say about drugs. The work is remote, hourly or per project, and it uses your drug knowledge rather than your dispensing hours.
These roles were open on Mercor‘s public feed on 2026-10-13, listed at the rates below:
| Role as listed | Listed pay | Hours and location as listed |
|---|---|---|
| Pharmacist / Pharmacy Technician, Medication Pronunciation Reviewer | $70 an hour | Remote. US pharmacy experience. Scores audio of spoken drug names against a rubric |
| Payer and Market Access Expert, Gross-to-Net and Formulary Strategy | $175 to $200 an hour | Remote, US based. About 10 to 20 hours over a 1 to 2 week pilot. Asks for 5+ years in market access or managed markets |
| Medical Expert (standing listing for health professions, including pharmacy) | $60 to $180 an hour | Remote. Range is what listings across the field have posted. Most of the work is written reasoning |
These are the rates the listings state, not what anyone will earn. Listings open and close often, and a listing is no guarantee of work.

The market access role is a good example of managed care experience paying off: it asks pharmacists to judge formulary tiers and rebate assumptions by drug class.
Mercor screens with an AI interview of about 20 minutes. It offers a free practice interview and lets you retake the official one up to three times. Our AI interview practice page helps you rehearse.
Doctors and other clinicians have their own version of this work. See medical AI trainer jobs.

How to Move Into Remote Pharmacy Work
- Pick the path that matches your background. Hospital experience suits remote order verification, and claims or prior authorization exposure suits managed care.
- Check which state licenses each role needs before you apply.
- Meet the field. AMCP suggests that practicing pharmacists switching into managed care go to AMCP events, job shadow, and consider a non-traditional residency or fellowship.
- Treat an entry-level managed care role as a foot in the door, as AMCP puts it.
