Medical AI trainer jobs pay doctors, nurses and other clinicians to write realistic cases and grade how AI models reason about them. The work is remote and paid by the hour or by the task, and most listings want a current license and someone still seeing patients.
I do AI training work myself. The listings below are the ones open on Mercor‘s public feed on 2026-10-13, with what each one asked for.
What Medical AI Trainers Do
Current physician and clinician listings describe these tasks:
- Writing a clinical case from your own practice, with the presentation, labs and imaging, plus the reference answer a model is graded against.
- Grading a model's diagnostic reasoning: whether the differential is ordered sensibly, whether it anchors too early, and whether the workup follows guidelines.
- Flagging advice that is defensible in a textbook and wrong for the patient in front of you.
- Reviewing AI-written notes, progress notes and discharge summaries for omissions, invented details and clinical errors.
- Comparing two AI responses head to head and saying which is clinically sounder.
- Labeling patient charts so a model learns to spot a specific condition.
Most of the job is explaining your reasoning in writing. One standing listing says so directly.

Who Gets Hired
Requirements differ by listing, and they are specific. Here is what the open roles asked for:
- Physician roles ask for an MD or DO, often with board certification and an active, unrestricted US license. Several want 2 or 3 years after residency and current practice.
- Some projects want one specialty, such as oncologists in or past a hematology and oncology fellowship, or cardiologists with heart failure experience.
- One standing physician listing welcomes final-year residents and fellows.
- Some documentation projects run multilingual groups and ask for a second language at an advanced (C1) level.
- One nurse practitioner listing asked for 4 or more years as an NP or APRN.
- An RN listing asked for recent inpatient bedside experience and at least 10 hours a week.
Most of these roles were limited to people based in the US.

Some paid studies pay a flat amount instead. A 30-minute survey for board-certified urologists and medical oncologists was listed at a one-time payment on the same day.
How Screening Works
The physician listings describe a short, structured process:
- Upload your resume and confirm where you work from.
- Take an AI interview. Mercor says most take about 20 minutes, and one physician listing describes a 25-minute conversational interview about your background.
- Pass credential checks. One listing verifies license status, residency completion and current practice setting.
- Wait to be matched. Standing pools invite you to a specific project when one fits, which the listing says can take a week or several months.
Mercor offers a free practice interview that does not count, and you can retake the official one up to three times. Our AI interview practice page lets you rehearse out loud.
Ethos, the other platform DigiNo covers, runs expert projects as consultations, surveys, written reports and review or annotation work. Its terms let you decline any project.

Keep Patients Out of Your Cases
Some tasks ask for a case from your own practice. Under HIPAA's Safe Harbor method, de-identifying health information means removing 18 types of identifiers, including names, places smaller than a state, dates more specific than the year, and record numbers. Write cases that no one could trace back to a real patient. This is not legal advice.

Check Your Contract
Employment contracts handle outside work differently. A 2012 article in The Hospitalist notes that some employers ban outside activities altogether, some allow ones that do not interfere with your duties, and some keep the right to approve them or claim the income.
Residents have their own rules. The ACGME requires that moonlighting not interfere with training, counts it toward the 80-hour weekly limit, and bars PGY-1 residents from moonlighting. Ask your program how it treats AI training work.

Other Paths for Clinicians
AI training is one of several ways doctors use their training outside the clinic. Our guide to non clinical jobs for physicians covers the full-time routes, and side hustles for doctors covers the part-time ones.
