A bilingual therapist provides counseling or therapy in more than one language, so clients can talk about the hardest things in the language they think and feel in.
Spanish-language mental health care has been getting harder to find in the US, and clinical skills now open a second door too: remote AI training work for mental health professionals.
What a Bilingual Therapist Does
- Delivers therapy in the client's preferred language, without an interpreter in the room.
- Works in the client's primary language, which the American Psychiatric Association says improves communication and lowers barriers to treatment.
Why Bilingual Therapists Are in Demand
About 23% of people in the US aged 5 and over speak a language other than English at home, according to the US Census Bureau 2024 American Community Survey. Spanish accounts for about six in ten of them.
Care in Spanish has not kept up. A 2022 study in Psychiatric Services found the share of US mental health facilities offering treatment in Spanish fell by 17.8% between 2014 and 2019, with declines in 44 states, while the Hispanic population grew in every state.

Who Can Become a Bilingual Therapist
- A clinical license in your state. The Bureau of Labor Statistics says all states require mental health counselors to be licensed; common licenses include LCSW, LPC, LMFT and psychologist.
- Fluency in a second language strong enough for clinical conversations, not just everyday ones.
Some listings name the level they want. Several clinical AI listings seen on 2026-10-08, for example, asked for a second language at an advanced (C1) level or above.

Where Bilingual Therapists Work
Bilingual therapists work in the same places as other counselors. The largest employers of mental health counselors, by share of jobs, are:
| Setting | Share of counselor jobs |
|---|---|
| Offices of other health practitioners | 20% |
| Outpatient mental health and substance abuse centers | 16% |
| Individual and family services | 15% |
| Residential mental health and substance abuse facilities | 8% |
| Hospitals | 7% |

Remote Work Beyond Therapy: AI Training
AI labs need clinicians to judge whether AI conversations about mental health are realistic and safe.
On 2026-10-08, the public feed on Mercor listed a remote, hourly role for clinical mental health experts to compare pairs of simulated clinical conversations for realism. It welcomed backgrounds from clinical psychology and counselling to clinical social work and marriage and family therapy. The work does not involve treating clients. Other clinical AI listings in the same feed were multilingual cohorts that required a second language.
